The Process

Making a referral

Whilst we will accept self-referrals, we would strongly advise you ask your GP to refer you, as they can then provide details of your previous medical history that may be relevant. This will help ensure a more efficient and comprehensive assessment, and help avoid delays in providing your completed report and treatment advice. You or your GP can make a referral by contacting our admin team on (07818262986 and admin@chmclinic.co.uk).
